We provide support to young people aged between 12 and 17 who do not require out-of-home care but may need help in managing their lives.
Our adolescent support program works with young people living in many different situations. They can be living at home with their family, in accommodation they have found themselves, in transitional housing or living in the out of home care sector.
We receive community and government referrals for young people who may require some additional support. They’ll meet with a Wesley case manager to determine together their goals for the future and support requirements. We determine their immediate and ongoing needs, and provide ongoing advice and supervision.
This program is available to:
- Young people referred from the Department of Health and Human Services
- Young people in need of support in regards to education, health or behavioural management
- Families with young people who need help to ensure the family unit stays together in a safe environment.
